FSC to-do list: submit fees, medical consents, sign release form

There are three items we need to cross off our to-do list before we can complete sign-in for the 2016 Friendship Cup tournament.

Submit tournament entry fees

  • If your daughter is playing in the tournament, the amount due is $60 per player. If your daughter isn’t playing in the tournament, the amount due is $12 per player (other non-tournament Jr. Academy expenses). You can write a check to our treasurer, Heather Prymak, and deliver to her before a practice, or to her or Ward after a practice. Link:  tournament fees and other expenses

Submit medical consent forms

  • We’re required to present individual player medical consent forms for 2015-2016 (along with team release of liability forms) during tournament check-in on Friday, May 20th. We collected some of these at our orientation meeting and Academy Camp last summer, but many have still not been submitted. If you have not yet submitted a medical consent form, please deliver to Ward after a practice the week of May 9th or May 16th. If you have access to a scanner, you can scan the form and e-mail Ward the digital file.
  • As of May 19th PM, we have received medical consent forms for all players. Thank you!

Sign team release of liability form

  • We’ll collect signatures for team release forms before / after practice the week of May 9th and May 16th. This is separate from the individual medical release form mentioned above. If you aren’t able to attend practice to sign, please authorize another parent to sign as your proxy.
  • As of May 19th PM, we have collected signatures for all players. Thank you!
